冷冻沉淀品

冷冻沉淀品(英语:Cryoprecipitate,简称英语:Cryo)是一种由血浆制成的血液制剂[1]。用于治疗凝血病,如:先天性纤维蛋白原缺乏症英语Congenital afibrinogenemia或因产科出血英语Obstetrical bleeding弥漫性血管内凝血造成的纤维蛋白原过低现象[2] [3] [1]。虽然过去被用于治疗A型血友病英语Haemophilia A类血友病,但目前倾向使用已有的特定凝血因子浓缩物[4]。透过静脉注射,[5]注射前不需交叉配对英语Cross-matching,但可能的话,最好先进行ABO血型配对[4]。成人通常一次注射两个 5 单位的剂量(共十单位)[4] [3]

副作用包括可能发烧过敏急性溶血英语Acute hemolytic transfusion reaction急性肺损伤和感染[4]。一般不用于泌尿道出血[1]。将新鲜冷冻血浆在4°C解冻,然后离心收集沉淀物[1] [4]。再将沉淀物溶于少量的残留血浆中(通常是 15 至 30 毫升),并再冷冻储存[4] [3]。内含因子 VIII英语Factor VIII类血友病因子纤维蛋白原纤连蛋白因子 XIII英语Factor XIII [3]

冷冻沉淀品由Judith Pool在1964年制造[6] [7]。它是第一个治疗A型血友病的药物, [1]列入世界卫生组织基本药物标准清单[5]。可保存于-25 °C,长达一年半[3]。在室温解冻后下,应在 4 小时内使用[3]。在英国国民保健署一剂5个单位的冷冻沉淀品费用约为 181 英镑[3]。它比纤维蛋白原浓缩物英语Fibrinogen concentrate便宜[4]
